Bottled gases are restricted whether or not the gas is hazardous due to the compressed nature of the cylinder. This restriction covers small disposable as well as large metal cylinders. All gas purchased through the university must go through General Stores whether it is compressed or liquid gas. Please reach out to General Stores at 801-581-8671 to complete the purchase.
Ethyl alcohol may only be ordered from the University of Utah’s General Stores via the “Ethyl Alcohol Order Form” in UShop’s Internal Suppliers Forms or via the General Stores webpage. When ordering ethyl alcohol, the department will obtain a Tax-Free Alcohol Report, from General Stores, delivered with their first order. When properly filled out and signed by the department head or research supervisor, the Tax-Free Alcohol Report, along with an order, must be submitted to General Stores to reorder. The requesting department will be issued a new Tax-Free Alcohol Report form with each request. A separate form must be used for each size and proof of ethyl alcohol. These records are subject to federal audit and should, consequently, be carefully and thoroughly maintained.
According to State of Utah law and University of Utah procurement policy, any hazardous or radioactive materials, controlled substances, or select and biological toxins being delivered to or used on campus must have approval from the Office of Environmental Health & Safety and or the Radiation Safety Office.
Controlled substance orders must be submitted on a requisition with the applicable DEA license that is current and has the handler of the controlled substance listed.
Radiation source (i.e. radioactive materials, radiation-producing equipment such as x-rays, lasers or other non-ionizing radiation equipment) orders must be submitted to Purchasing on a requisition. Radioactive source orders to be placed for next day delivery must be received by Purchasing by 10 a.m.
Note that Purchasing must still process materials with low radioactivity.

Select Agents and Biological Toxins are regulated by the Centers for Disease Control and Prevention. All research activities which possess, use, or transfer biological agents and toxins that have been determined to have the potential to pose a severe threat to human health, animal health, plant health, or to animal and plant products are restricted. All principal investigators must be approved by the University of Utah Institutional Biosafety Committee (IBC) prior to possessing these materials. Contact the Biosafety Office in Environmental Health and Safety at 801-581-6590 or email for information on how to complete the IBC registration.

All orders in UShop must be placed on the OCM form titled “Office of Comparative Medicine Order”. Please contact OCM 801-581-6430 with any questions.
Many equipment and supply items may incur customs clearance charges if procured from a foreign country. These items will be held at U.S. Customs offices for clearance if the paperwork is not in order. Even free items will incur shipping and import charges.
There is only one company the university contracts with for customs clearance. Place internationally ordered items on a requisition. When submitting the requisition please ensure the “Yes” button is selected in the Purchasing Department Details section with a note explaining importation is needed, so it routes to purchasing for review.
Recruitment Advertising requests for full-time staff and faculty positions must be submitted to Purchasing on a requisition. Staff position requisitions should reference the Human Resources Specialist name, job number and placement information. Faculty recruitment requisitions require the VP’s signature. Please note that your request cannot be processed without the required authorization.
